Role Summary:
- Working as part of the South West Team alongside NHS and community services.
- Ensuring home and hospital-based tailored support using a needs-based assessment framework.
- This is a maternity cover contract up to 1 year with the flexibility of a hybrid work arrangement.
Role Requirements:
- A recognised Social Worker qualification is required and current registration with Social Work England or equivalent.
- Work experience in a social care setting with children and their family is essential, preferable within an NHS setting.

The position is a maternity cover contract for up to 1 year, offering hybrid working options.
