More than 500,000 people die each year in the UK. Half of these occur in hospital many without medical reason to be there. Digital Care Planning supports people to live well with a diagnosis until death engaging people in long-term health care planning. We encourage people to face the end and plan as best they can for themselves and those they leave behind.
