Since 2016, we have supported people in a variety of ways, including delivery of NHS Well-being Programmes, Dementia & End of Life Programme, Moor Social Events, Befriending and Digital Champions work. Moorland Community Caring's aim is to help people to live independently – to connect them to their community, whether this means via other organisations or with our wonderful volunteers, to gain confidence to lead independent and fulfilled lives. We achieve this through listening and helping to create a support plan tailored to an individuals needs. Moorland Community Caring is part of the South Devon Well-being Partnership, delivering well-being programmes, individually tailored to a patient’s needs. Working in collaboration with GPs, District Nurses, Mental Health practitioners, Social Care colleagues and Social Prescribers, we engage with patients for 12 – 20 weeks, helping them to improve their well-being. This may be addressing loneliness, anxiety, diet, physical illness, budgeting and more.
